Routine appointments
You can request a routine appointment with a clinician for something that is non-urgent. We will contact you within 2 working days. Please use the form below to request your appointment.
Urgent appointments
You can request an urgent appointment with a clinician for any urgent queries. These will be clinically assessed on the day you send the form. Please ensure you are contactable until 14:00.
If you experience difficulties completing the online form, you can also phone us on 01723 360098 and choose option 5 for a member of our Practice Support Team to help.
Please request an interpreter at the time of booking if you cannot communicate in English.

We will use your answers to choose the most suitable doctor, nurse or healthcare professional to help you.
Evening and weekend appointments
Working in collaboration with extended access, we are pleased to offer access to GP, nurse and HCA appointments on Monday evenings and various Saturdays throughout the year.
To book appointments, simply ask the receptionist who will arrange an appointment at a time and location that suits you best. This will not be at your usual practice, but the GP or healthcare professional who sees you will have access to your medical record and can issue prescriptions if required and deal with other matters appropriately should the need arise.

However you choose to contact us, we may offer you a consultation:
- by phone
- face to face at the surgery
- by text
Appointments by phone or by text can be more flexible and often means you get help sooner.
Cancelling or changing an appointment
To cancel your appointment:
- use your NHS account (through the NHS website or NHS App)
- use the GP online system: SystmOnline
- phone us on 01723 360 098 during opening times
If you need help when we are closed
If you need medical help now, use NHS 111 online or call 111.
NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.

Please tell us:
- if there’s a specific doctor, nurse or other health professional you would prefer to respond
- if you would prefer to consult with the doctor or nurse by phone, face-to-face or by text
- if you need an interpreter
- if you have any other access or communication needs
Home visits
If you feel that you are not able to come to the practice, please phone 01723 360098. Where possible, please call before 10am. The practice will call you as soon as possible and arrange a visit if necessary.
Home visits are only available for patients who are housebound because of illness or disability.
These visits normally take place between midday and 2pm, Monday to Friday.
We request that you attend the practice for appointments if at all possible because we have better facilities at the surgery. This also makes maximum use of the doctors’ and nurses’ time.
