Summary Care Record Opt Out

A Summary Care Record (SCR) is an electronic summary of key information from your GP medical record, accessible to authorised healthcare professionals outside your usual GP practice. It provides essential details like current medications, allergies, and any adverse reactions to medications, helping healthcare staff provide safer and more efficient care, especially when you need treatment away from your regular GP. 

It also includes your name, address, date of birth and your unique NHS number to help identify you correctly. Additional information can also be added to your summary care record.

Use this service to opt out of your summary care record.

  • are registered at the surgery

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of
