Nurse Practitioner

We are looking to appoint a Nurse Practitioner who can deliver a high standard of patient care using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base managing clinical caseloads, dealing with presenting patient’s needs in a primary care setting and be a proactive team member of the Advanced Clinical Team at Brook Square Surgery.

Practice Nurse

We are looking for a motivated and forward thinking Practice Nurse to join our friendly Clinical Team.

Duties to include:

  • Chronic Disease Management
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition
  • Work with patients in order to support adherence to prescribed treatments
  • Implement and participate in vaccination and immunisation programmes for both adults and children
  • Meet the needs of patients presenting for opportunistic wound care
  • On occasion the post –holder will be required to visit patients in their own home eg to conduct chronic disease reviews

This list is not exhaustive - the Practice Nurse’s duties are many and varied.

The successful applicant will have a relevant nursing/health degree or equivalent and have qualifications in Diabetes, Insulin Initiation, COPD, Asthma, Sexual Health and Cytology.

 

The post is a permanent position with full and part-time positions available. You will need to have a flexible approach to your work in order to fit with the demand of the practice
